Allied S45C 4"  Three Jaw Gear Puller/Remover. This has a fourth slot for an additional jaw but I think it is for adjusting the three jaws for a better pulling experience. This tool has earned it's money over the years judging by all the grease stains and shop dings but it remains in great shape with years of service left. Please view pictures for quality and condition.
